What is paediatric podiatry and which foot problems does it see in children?
Paediatric flat foot is a frequent reason for consultation in primary care, and a minority of flat feet may hurt and limit walking.1
Parents are often worried about the shape of their children's feet, but in most cases these are variants of normal development and nothing needs to be done. Around the age of 3 or 4 it is normal for a child to have hardly any plantar arch, and if the arch forms when they stand on tiptoe, the foot is flexible and is normal even if it looks flat. The definitive footprint is established around the age of 6 or 7.2
Flat foot is the frequent reason for consultation covered by the sources cited, and that is why it is the problem this page develops. Those sources do not cover toe walking, toe position, nails, warts or growth-related heel pain, so this page does not deal with them.
When does a flat foot in a child need treatment?
A flat foot in a child that is flexible and does not hurt does not need treatment.2 As long as the feet are flexible and have no limitation of movement, no treatment is needed, and flexible flat foot usually disappears by puberty. If the foot does not hurt, in general no insoles are needed, and the use of insoles has not been shown to make the arch develop better.2 Correcting it as early as possible, therefore, is not what this source indicates.
On children's insoles, the Cochrane review (with children with flexible flat foot) concludes that their effect compared with footwear on pain, function and quality of life is uncertain, with low or very low certainty. In the absence of pain, there is no evidence to support expensive custom-made insoles in healthy children with flexible flat foot.1 There is also no optimal strategy or consensus for using foot orthoses in children's flat foot.1 The sources cited do not detail the adverse effects of insoles.
The exception is juvenile idiopathic arthritis. The Cochrane review found that, in children with that condition, insoles may improve pain and function compared with usual footwear, with a minimal difference between expensive custom-made insoles and generic ones. This applies only to those children, not to the rest.1 Its authors recommend focusing attention on paediatric foot conditions that cause pain, limit function or reduce quality of life.1
What footwear do young children need?
Young children should never wear rigid shoes or shoes with hard soles: footwear should be flexible and wide, light and with a flexible sole, with reinforcement at the heel and not covering the ankles.3 That is why "the firmer and harder the better" is not what the AEPap recommends.
The front part of the shoe should be wide and high and should not squeeze the toes, which must be able to move, and the rigid part of the sole should not reach the tip. Infants only need a comfortable, flexible shoe, and what advertising says about putting them in hard footwear with insoles so that they walk better is false.3 The sources cited do not detail specific adverse effects of unsuitable footwear.
When should you take a child to the podiatrist or the paediatrician?
Seek advice about a flat foot if there is pain or lack of flexibility.2 The sources cited do not set an age for the first visit or how often to have check-ups, so they are not given here. These are the reasons for consultation that they do list:
Options compared
| Option | What the sources say | Points to bear in mind |
|---|---|---|
| Watching without treating a flexible flat foot that does not hurt | If the feet are flexible and have no limitation of movement, no treatment is needed, and flexible flat foot usually disappears by puberty. | Pain and lack of flexibility need to be watched for, as they are the reasons to seek advice. |
| Expensive custom-made insoles | In the absence of pain, there is no evidence to support their use in healthy children with flexible flat foot. | The sources cited do not detail adverse effects, and the evidence does not favour the paid option if there is no pain. |
| Custom-made or prefabricated insoles versus footwear | Their effect on pain, function and quality of life is uncertain, with low or very low certainty. | A minority of flat feet may hurt and limit walking, and there is no consensus on when to use orthoses. |
| Hard footwear with insoles for babies | The AEPap says that what advertising claims about it is false. | Young children should never wear rigid shoes or shoes with hard soles. |
Signs to seek care without waiting
- Pain in a child's flat foot.
- A foot that lacks flexibility.
- Any foot deformity in a newborn: it should be assessed by the paediatrician, who evaluates flexibility and considers treatment or referral to a paediatric orthopaedic specialist.

Frequently asked questions
Do all children need insoles?
No: if the foot does not hurt, in general no treatment or insoles are needed, and the use of insoles has not been shown to make the arch develop better.
Do babies need shoes?
Infants only need a comfortable, flexible shoe.
Do insoles help children with juvenile idiopathic arthritis?
Only in that group: in children with juvenile idiopathic arthritis, insoles may improve pain and function compared with usual footwear, according to the Cochrane review. It does not apply to children without that condition.
