What is a plantar wart or papilloma and how is it spread?

A plantar wart (or plantar papilloma) is a benign lesion on the sole of the foot caused by the human papillomavirus (HPV).12 They are very contagious.1

The incubation period ranges from 2 to 20 months; on the foot they usually appear between 6 and 18 months after infection, and they mainly affect teenagers aged 12 to 25.1

The hygiene measures listed by the Consejo General de Colegios Oficiales de Podólogos (CGCOP) to prevent infection are these:1

  • Do not walk barefoot.1
  • Do not share footwear or socks.1
  • Do not touch the lesions.1
  • Use a towel only for the feet.1
  • Check your feet, because other lesions can go unnoticed.1

How do you recognise a plantar wart and how does it differ from a corn?

A plantar wart is recognised by being on the sole of the foot, by the fact that it can be very painful2 and, according to the NHS, by having small black dots under the hard skin.3

The CGCOP sets out the difference from a corn: when the lesion is scraped, blackish bleeding dots (papillae) can be seen, which a hardness or plantar callus does not have, and a corn hurts more under pressure than when pinched.1 Do not do this yourself: handling it incorrectly can spread the lesion.1

The sources cited do not describe laboratory or imaging tests to confirm it.

How does a podiatrist treat plantar warts?

The podiatrist treats plantar warts with physical or chemical therapies. Cryotherapy, laser and electrocoagulation cause pain and a physical burn that is needed to remove the lesion, with dressings every 48 to 72 hours.1 The duration depends on the extent and depth of the lesion and on the patient's follow-up, and it can be weeks or months; they rarely become infected.1

MedlinePlus notes that plantar warts and warts near the nails are harder to cure and can come back; if they persist, a professional may recommend cryotherapy, electrocautery, laser or other options.2

Waiting, pharmacy or clinic: what the evidence says about each option

Warts usually go away on their own, although they can take months or even years,3 so the evidence cited does not require paid treatment in every case.

  • Waiting: MedlinePlus says they are almost always harmless and go away on their own in about 2 years.2 In the meantime they can hurt2 and spread.1
  • Pharmacy products: they can take up to 3 months, irritate the skin and do not always work.3 If you have diabetes or a weakened immune system, ask first.2
  • Cryotherapy, laser or electrocoagulation: pain, a physical burn and dressings every 48 to 72 hours, for weeks or months.1 They can come back.2 The sources cited do not detail the effectiveness of each technique.
  • Chemical therapies: the CGCOP mentions them alongside the physical ones,1 but the sources cited do not detail their risks.

Why it is best not to remove it at home

MedlinePlus asks you not to try to remove a wart yourself, with any method.2 The CGCOP adds that handling it incorrectly can spread the lesion to other areas.1

When should you see the podiatrist or the doctor about a wart?

See the doctor or the podiatrist about a wart if there are signs of infection or bleeding, if you have diabetes or a weakened immune system, or if it is very large, painful or comes back.23

  1. Signs of infection or bleeding: MedlinePlus asks you to seek advice.2
  2. Diabetes or a weakened immune system: MedlinePlus asks you to seek advice if warts have appeared.2
  3. It bleeds or changes appearance, is very large or painful, comes back or worries you: the NHS says to seek advice; a large or very painful wart may be referred to a podiatrist.3
