What is a biomechanical gait study and who does it?

A biomechanical gait study is a protocol-based examination of the lower limbs which, according to the guide of the General Council of Official Associations of Podiatrists (CGCOP), is the first step of the clinical and biomechanical assessment.1 That guide was published to give the podiatrist uniform criteria for the examination and biomechanical assessment of patients, and it is aimed at the podiatrist.1

The basic examination described in the guide includes the joint, muscle and neurological assessment, the dynamic assessment (by direct observation or camera recording from behind, in front and from the side) and the baropodometric study or footprint.1 Specific tests are chosen according to the patient.1 The cited sources do not support the superiority of any particular equipment.

Pronated, supinated and neutral gait

People talk about pronated, supinated and neutral gait. What the evidence supports is limited to pronation, and it refers to foot posture, not to a type of gait.

A systematic review with meta-analysis of 21 prospective studies (6,228 participants) found strong evidence that a foot with a pronated posture is a risk factor for medial tibial stress syndrome, and very limited evidence that it is one for patellofemoral pain. The effect sizes were small (0.28 to 0.33) and it found no relationship with other injuries assessed.2

Having a pronated foot posture is not a diagnosis: it is a risk factor with a small effect for those injuries.2 The cited sources give no data on supinated or neutral gait.

What is the study like, step by step?

The biomechanical gait study follows the protocol-based examination described in the CGCOP guide, with a systematic assessment of the lower limbs and specific tests according to the patient.1

  1. Joint, muscle and neurological assessment: part of the basic biomechanical examination.1
  2. Dynamic assessment: by direct observation or camera recording from behind, in front and from the side.1
  3. Baropodometric study or footprint: also included in the basic examination.1

If you play sport and only want to know which trainers to buy, bear in mind what the evidence says: prescribing trainers according to foot posture probably makes little or no difference to injuries, although the trials were done in a military population and the result may differ in recreational runners.3 You can ask what decision will be made with the result before booking.

If you have repeated pain and doubt that you need a study, or fear that you will be offered insoles you do not need, ask what decision will be made with the result: the study is an assessment and there is no evidence here that it leads to prescribing insoles. The cited sources do not detail the interview, the length of the visit, the format of the report or the drawbacks of the study.

Who needs a gait study and who does not?

The cited sources do not identify precisely who benefits from a study; this is what they do report.

  1. The biomechanical examination is the first step when a patient is assessed clinically for a lower limb problem, with specific tests according to the case.1
  2. Pronated posture is a risk factor with a small effect for medial tibial stress syndrome, with strong evidence, and for patellofemoral pain, with very limited evidence. No relationship was found with other injuries assessed.2
  3. Choosing trainers according to foot posture probably makes little or no difference to lower limb injuries (moderate certainty, three trials in a military population).3
  4. Types of shoe in general: the certainty of the evidence is very low to low, so their real effect on injuries is uncertain.3

On choosing trainers, the Cochrane review concludes that prescribing them according to foot posture probably makes little or no difference to lower limb injuries.3 For anyone with recurrent pain in the foot, knee or back, or repeated injuries when running, the cited sources do not show that the study is necessary or that it solves the problem; they do describe the biomechanical examination as part of the clinical assessment.1 If the pain is severe, you cannot walk, or the foot has changed shape or is hot, swollen and painful, the NHS says to seek urgent care.45 This information does not replace a consultation with a health professional.

The association between pronated posture and injuries has a small effect and is limited to a few injuries.

Frequently asked questions

How can I find out what type of gait I have?

The cited sources do not explain how to tell the types of gait apart on your own. The CGCOP guide, aimed at the podiatrist, includes in the biomechanical examination the dynamic assessment and the baropodometric study or footprint.

Where can I have a gait study near me?

This explanation does not list specific centres. The CGCOP guide on the examination and biomechanical assessment of patients is aimed at the podiatrist.

What should I ask before booking a gait study?

You can ask who carries it out (the CGCOP guide is aimed at the podiatrist), what it includes (joint, muscle and neurological assessment, dynamic assessment and baropodometric study) and what specific decision will be made with the result, because prescribing trainers according to foot posture probably makes little or no difference to lower limb injuries.
